Application Forms

These official SSA forms and pages may be useful when starting a Social Security Disability or SSI claim. Some forms can be completed online, while others may need to be printed, mailed, or provided to a local Social Security office. If you have already received a denial, make sure you are using the correct appeal form rather than starting over with a new application.

Release of Information Form

Social Security may need written authorization before medical providers, hospitals, schools, employers, or other sources can release information for a disability claim. The SSA-827 is the standard authorization form used by Social Security to request medical and other records needed to evaluate disability.

Do not sign blank or incomplete forms. Make sure any release form is completed, signed, and dated before it is submitted. Consent for Release of Information

Appeal Forms

 If your Social Security Disability or SSI claim has been denied, the denial notice should explain your appeal rights and the deadline to appeal. The form you need depends on the stage of the case. A claimant appealing an initial denial may need a Request for Reconsideration, while a claimant appealing a reconsideration denial may need a Request for Hearing by an Administrative Law Judge.

Disability appeals often require more than one form. In many medical disability appeals, SSA may require an appeal request, a Disability Report - Appeal, and an authorization to obtain records. If you are unsure which form applies, review your denial notice carefully or contact us before filing.

Work History and Function Reports

Social Security may ask for additional forms about your past work, daily activities, symptoms, and how your medical conditions affect your ability to function. These forms can be important because they help explain what you did in past jobs and what limitations you experience in daily life.

Take your time when completing these forms. Be accurate, specific, and consistent with your medical evidence. Do not exaggerate, but do not minimize your symptoms or limitations either.
